St. George on <strong>Wednesday from 12:30pm to 4:00pm.<\/strong>\u00a0 Suggested donation is $5.00 round trip for all transportation provided, for clients 60+.\u00a0 Transportation is provided on a first come, first served basis.\u00a0 So, the earlier you call the better your chances of getting the requested assistance.\u00a0 Rides are not available on Holidays or when the Center is closed.<\/p>\n<p><strong>To schedule transportation<\/strong>, please call the Hurricane Active Life Center at<strong>\u00a0<i>435-301-7280.<\/i><\/strong><\/p>\n<ol>\n<li><strong>TO SCHEDULE A RIDE \u2013 LEAVE A VOICEMAIL ANYTIME<\/strong>\u00a0with the following information\n<ul>\n<li>First and Last name, telephone number and home address<\/li>\n<li>Date and time you would like to be picked up at home<\/li>\n<li>Destination name and complete address<\/li>\n<li>Appointment time (if applicable)<\/li>\n<li>Time you would like to be picked up and taken home<\/li>\n<li>Any special conditions including mobility equipment\/ personal assistant, etc.<\/li>\n<\/ul>\n<\/li>\n<li>We will call you back if there is an issue with your request.<\/li>\n<li>Request rides at least <strong><u>ONE<\/u><\/strong> business day in advance\n<ul>\n<li>First Come \u2013 First Served<\/li>\n<li>Medical appointments take precedence over other rides<\/li>\n<\/ul>\n<\/li>\n<li><strong><u>SUGGESTED DONATION FOR 60+<\/u><\/strong><strong>: $5.00\u00a0 \u00a0\u00a0<\/strong><\/li>\n<li>Driver can <strong>only<\/strong>\u00a0take\u00a0<strong>scheduled riders<\/strong>\u00a0to\u00a0<strong>scheduled destinations<\/strong>\u00a0(if your spouse is coming with you, you must include him\/her when requesting the ride)<\/li>\n<li><strong><u>BE READY!<\/u><\/strong>\u00a0The driver has a\u00a0<strong><em><u>15 minute<\/u><\/em><\/strong>\u00a0window on\u00a0<strong><em><u>either side<\/u><\/em><\/strong>\u00a0of the scheduled pick up time.\n<ul>\n<li>The rider shall be ready 15 minutes prior to scheduled pick up time and come out to the bus when the bus arrives<\/li>\n<li>The driver will wait a maximum of three minutes at pickup location for passenger. This will be strictly enforced to keep the drivers on schedule.<\/li>\n<\/ul>\n<\/li>\n<li><strong><u>CANCELLATIONS:<\/u><\/strong>\u00a0The rider is responsible for cancellation of scheduled rides as soon as possible so other people may be served. You can leave a voicemail to cancel.\n<ul>\n<li>To cancel a scheduled ride, please call 435-301-7280 (Please leave a voicemail if it is not during our normal operating hours) If you must cancel the day of your appointment you must leave a message before 8:00am.<\/li>\n<li>Failure to cancel your scheduled ride it will result in a No-Show being recorded to your customer record. Three no-shows may result in DAR service being suspended for a period of time.<\/li>\n<\/ul>\n<\/li>\n<li><strong><u>SERVICE ANIMALS ONLY:<\/u><\/strong>\u00a0Our policy on service animals follows the Americans with Disabilities Act.
